Condensation between double pane windows

My house is 18 years old. We have Anderson double pane casement
windows on every window in the house. The last couple of winters we are getting condensation on inner portion of the outside window. I'm not sure if this is the same as fogging as some threads have mentioned.

I assume this means a failure in the seal of those windows. Do I need to replace only the failed windows or should I replace all the 18 year old
windows? Can just the window portion be replaced (I'm not sure what the proper term is but I mean the double pane glass and frame that crank in and out not the entire frame that is attached to the house?)

You should be able to order new sashes (glass with wood frame) for the windows that require them. Take a good look at the condition of the rest of your window since it's not worth investing money if the rest of the window is in bad condition.
